The ODS Online Student Survey has been Extended One Week!

 

The deadline to complete the ODS Online Survey has been extended one week, until March 9, 2009! We have received very informative feedback so far, and would like to extend the opportunity for students to continue to voice their opinions about ODS’ services and accommodations.

 

This also means that there is still time to complete the survey for a chance to win 1 of 2 $50.00 check prizes! The drawing will now be held on March 9, 2008. The 2 winners will be notified on this day.

The staff at the OSU Office for Disability Services (ODS) takes much pride in the work we do and the services we provide.  We would like to get your feedback to help us assess and improve our services. All ODS students are invited to take the ODS Online Student Survey and tell us how we are doing!

 

This survey consists of modules that are brief and will not take much time.  We’d like everyone to complete the module, the ODS and OSU Climate and Accessibility Survey.  Then, we’d like you to complete only the remaining modules that apply to services that you actually use.  These surveys are short and will take only about 60 seconds each.

 

Your responses will be anonymous, but you can provide your contact information in the comment box if you wish. Once you have completed and submitted your modules you can print a confirmation showing your completion of the survey. Drop by the ODS office with this confirmation page, place it in the ODS Online Student Survey Entry Box at the front desk, and you will be entered into a drawing for one of two $50.00 checks!  The drawing will be March 2, 2009. The two winners will be notified on that day.
